Periodontitis is a complex dis ease associated with dysbiotic biofilms, characterized by loss of clinical attachment, periodon tal pocketing and bleeding on probing. Between 1990 and 2019, it was calculated that there were ap proximately 1.1 billion individu als affected by severe forms of periodontitis worldwide. In Italy, recent epidemiological studies have shown a prevalence of se vere periodontitis up to 40.78%, with an increased probability of developing the disease in smok ers and in subjects under 50 years of age. In response to these alarming da ta, the scientific community pro posed a more modern Classifica tion of periodontal and peri-im plant diseases and conditions, to allow a precise diagnosis, without neglecting even mild forms of periodontitis, identifying, through a Staging process, 4 Stages of in creasing severity and therapeutic complexity, and then introducing consequent guidelines to orga nize the anticipated therapies. In particular, Stage III and IV peri odontitis, due to the severe tissue destruction and need for complex treatment, represent not only a serious threat to the patient’s health and quality of life, exposing him to significant biological and economic costs, but also a de manding challenge for the dental team, obliged in the most serious cases to concert a complex multi disciplinary therapeutic approach. The present Dossier deals with some aspects of Stage IV peri odontitis: epidemiology and re lated impact on the patient’s systemic health and quality of life, the economic-financial im plications, the necessary multi disciplinary therapeutic ap proach, thus summarizing the cornerstones of the guidelines recently published by the Euro pean Federation of Periodontol ogy with the aim of explaining the appropriate treatment se quence. Conclusions underline the abso lute need to institutionalize these guidelines because the increas ing prevalence of severe peri odontitis requires a prompt re sponse aimed to standardise a predictable treatment.
