Oroantral communication is a pathologic pathway between the oral cavity and maxillary sinus. Oroantral openings may initiate infection in the maxillary sinus when not treated. Clinical and radiographic examination is very important in the diagnosis of acute odontogenic maxillary sinusitis. It is known in the light of studies that low-level laser therapy (LLLT) shows anti-inflammatory, analgesic, biostimulation and bactericidal effects in the applied area. The aim of this study is to treat a case of acute maxillary odontogenic sinusitis due to oroantral opening after tooth extraction with low-level laser therapy without surgical planning. Therapy was performed with a Fotona AT Fidelis biostimulation mode laser of 0.25 w power, 10 Hz frequency and 1064 nm length. Low-Level Laser Therapy (LLLT) was applied to the patient every day for 6 days. 30 s was applied to the tooth extraction site, 30 s to the intraoral maxillary sinus lateral wall and 30 s to the extraoral maxillary sinus anterior wall. The laser was applied with circular movements. At the end of the 7th day, clinical and radiographic examination showed that the opening was closed with epithelialisation in the socket and the maxillary sinus was completely healthy in CBCT images. According to the results of our treatment, LLLT is a successful alternative method in the treatment of acute odontogenic maxillary sinusitis after tooth extraction.
