The Role of Scintigraphy with Bone Radiotracers in Cardiac Amyloidosis

In appropriately selected patients with the suspicion of CA, once AL -CA is ruled out through negative tests for monoclonal component, cardiac scintigraphy with Tc-99m-DPD/PYP/HMDP allows a definitive diagnosis of ATTR-CA without the need for EMB or advanced imaging investigation. The implementation of cardiac scintigraphy in the diagnostic flowchart of CA has contributed to transforming the epidemiology of this condition, which is now increasingly recognized as a possible cause of HF. The screening of high -risk populations and early identification of CA are crucial to promptly commence patients on novel disease modifying therapies, which improve clinical outcomes when a sustained suppression or stabilization of amyloid precursor proteins is achieved.
