Pulse oximetry is a simple, safe and inexpensive assessment widely used for the diagnosis of cardio-respiratory disorders. However, some rare diseases cause low oxygen saturation at the pulse oximetry in the absence of cardiac or pulmonary dysfunction. The case described outlines the need to improve knowledge and take in consideration the rare causes of impaired pulse oximetry in order to avoid expensive and invasive procedures and ensure the best diagnostic approach. The case described is one of the most serious. Hence the need to know and take into account the rare causes of impaired saturimetry in order to avoid very expensive and invasive instrumental investigations and ensure the best diagnostic and therapeutic approach to these children.
