Osteodural abnonnalities are rare in von Recklinghausen neurofibromatosis, especially in the lumbosacral regio n. Based on the personal observations and a review of the literature we describe some ofthe main fonns of these osteodurallesions. Basically three forms may be distinguished, which seem to represent various degrees of tile same pathologic processo Their patilogenesis is still controversial, a complex regional mesodennal dysplasia, affecting bone, durai sac and nerve root sleeves is tile most likely explanation. Computed tomographyc mielograpily and MRI suggested that the I11cningoceles observed in von Recklinghausen neurofibromatosis are abnormally dilated elongated nave root sleeves and silould be distinguished from other meningeal cysts occurring in tile lumbosacral regio n. The most common clinica l presentation and the choice of management are discussed.
