Mal de Meleda (MDM) is a autosomal recessive palmoplantar keratoderma. It shows hyperkeratosis, erythema, hyperidrosis and fetid maceration. We describe the consequences of the MM in a patient after a working accident. T.S., with MDM, 65 years old, worker in fondry, suffers accident at work cutting his right hamd with a milling-jcutter. The lesion was treated with hand bandanging in flexion, pratice suitable in this case. Unfortunately the right hand in flexion was permanent. The principal aspects of the MM are the keratoderma and the less cutaneous elasticity that limit the movements of the hand and produce the continuous fissurate lesions. The reparation of the lesion with the hand bandanging in flexion mostly reduced the ability of relaxation of the cutaneous tissues and the prehensile ability. The medical-legal aspects include the professional responsibility and the working fitness. It is necessary besides to consider that the consequences have mostly increased the psychological condition of the subject and brought new impairments in the most common activities of the daily life. The property responsibility of the physician in civil field includes the reimbursement of the biological damage or of the reduction of the working ability.
