Marco Levi Bianchini, the first Freud’s translator in Italian, worked as a psychiatrist in the “Vittorio Emanuele II” Mental Hospital in Nocera Inferiore, a small city southern Naples. By means of the analysis of 247 case sheets, in this paper we have tried to evaluate the importance of theories of libido and of sexual etiology for the diagnosis of hysterical neurosis in the daily practice of the alienist Bianchini. At the same time, our interest was to evaluate if and how it is possible to find the “biolibido” concept in these case sheets, testing the Bianchini “distrust”, in his theoretical writings, concerning the psychosexual nature of libido. In the present work, we produce the most relevant results of our study. The Bianchini clinical writing has been compared with the one of other psychiatrist of the same period. Moreover, we try to suggest a thesis in which the distrust, expressed by the biolibido concept, is not simply the indication of a Bianchini “extraneousness” as regards of psychoanalysis; this distrust is presented as one of the first evidence of a problem which often recur in the history of our discipline: the repression of the childhood psychosexuality.
