As Cyber-Physical Systems (CPSs) are wide spreading in industry and in our lives, they are being requested to perform with the maximum possible availability and reliability. One of the most challenging domains is constituted by the healthcare devices, whose critical role in preserving human lives is obvious. On the other hand, complex clinical facilities (e.g., large hospitals, networks of connected smaller structures) must face with cost containments to ensure a sustainable service to the community. This chapter will describe the approach proposed in the COSYMA project, maximizing the continuity of service of a system and minimizing maintenance costs both through an integrated and intelligent management of reactive, proactive and predictive maintenance. The Smart Maintenance system, that COSYMA aims to define and implement, aims in a general sense to be a meeting point between the various stakeholders involved in production (Owner), use (Customer) and operations (Maintainer or 3rd Party) of a system. Enabling techniques of this approach are: edge-cloud continuum, formal models, machine learning, model-driven engineering, knowledge management and representation techniques.
