"Background. To increase the histocompatibility reliability several External Proficiency Testing (EPT) schemes have been established within Europe. All laboratories, applying for accreditation by the European Federation for Immunogenetics (EFI) are obliged to take part in standardization trials for Human Leukocyte Antigen (HLA) typing and to participate in EPT program(s) to cover all techniques in use for accredited activities. The success of regional trials prompted us, as also requested from Mayor Institute of Health (ISS) and Italian National Transplant Center, to expand the activity to the Southern Italian regions, involving Campania, Calabria, Puglia and Sicilia. In 1996, the Regional Reference Laboratory of Transplant Immunology (LIT) of Campania started to the Quality Control for Regional HLA typing labs (CQR) working in the field of Organ and\/or Stem Cells Transplantation. Information and invitation letters were sent to 24 laboratories of Southern Italy involved in Class I and Class II HLA typing for the EPT. . Aims. Participation in external proficiency testing workshops provides the opportunity to verify HLA typing results (performed by serology and at the DNA level) of a given laboratory via comparison with the other participating institutions and the analysis of discrepant, erroneous typing helps in understanding their causes and elimination.. Methods. Routinely, different techniques were performed: Complement-Dependent Cytotoxicity (CDC), DNA low resolution, and since 2009, high resolution only for DRB1*. All samples were selected to cover all the most common antigens; each laboratory has been given a lab code. Since 2000, we have adopted the guidelines of EFI and ISS for HLA typing EPT.. Results. Most labs showed problems with HLA-Cw and DQ locus typing; They could not type antigens of these loci. So, from 1996 to 2010 all the data have been analyzed by LIT and the results discussed versus the consensus typing. All participants have received the summary of the appropriate certificate, necessary for EFI accreditation. . Conclusions. It has believed that EPT participation not only reflects the need of labs to participate in a workshop for EFI accreditation, but also plays an important educational role prompted by technological improvement."
